Registration Fees
Registration Fees for the Perioperative Echocardiography Review Course is:

| * | Registration (after August 1st) | $800 |
Fees Include: Admission to all classes, syllabus and all other course materials, breakfast buffet and lunch are also included in the registration cost.
Refunds: A refund of the registration fee, minus a $75 processing fee will be allowed when requested in writing and postmarked no later than August 1, 2009. No course registration refunds will be issued thereafter.
Confirmation of registration will be sent via e-mail. A registration receipt and any additional information will be e-mailed to you at the address provided.
